What do I need to enroll my child at Cameron?
Kindergarten - 8th Grade:
- Child's ORIGINAL Birth Certificate (copies are not accepted)
- Proof that address is within Cameron's attendance boundary. Bring 2 of the following types of acceptable identification:

- Utility bill (must be current)
- Driver's License/State ID
- Lease with name
- Court documents (must be current)
- Illinois Department of Public Aid card
- Physical Form (including record of vaccination)
- Dental Form
- Driver's License/State ID
- Transfer documents from previous school (if applicable)
Preschool:
1. Scan the QR code or apply online at: chicagoearlylearning.org
2. After confirmation of placement at Cameron, please bring:
- Child's ORIGINAL Birth Certificate (copies are not accepted)
- Physical Form (including record of vaccination)
- Dental Form
- Driver's License/State ID
- Transfer documents from previous school (if applicable)
* Preschool students do not need to live within Cameron's attendance boundary
